The Field Problem Report (FPR) form should be utilized to report and document any field related issues. This enables a direct flow of data back to Engineering, Broadcast and Supply Chain to identify signal impairments, manufacturing defects and advanced service failures.

FPR should be used to report:

-Broadcast Issues
-Set-Top-Box Problems
-Hardware Issues
-Advanced Service Products
-Drop Material Issues
-Problems identified with approved tools or AIMs
-Miscellaneous field-related problems such as shipping damage, unapproved materials, or items that may adversely affect the quality of installation.
